Assessment:Examine:  IV catheter placed.  0.15 cc butorphanol given slow IV. Adequate sedation.An abdominal ultrasound was performed. The report is as follows:1. Urinary Bladder - all well2. Medial iliac & hypo gastric Lymph nodes: ok3. Adrenal glands: ok - not enlarged4. Left and right kidneys showed good corticomedullary differentiation. 5. Spleen: The spleen was normal in size, shape and echogenicity.  6.  The pancreas was normal in appearance. 7. The stomach - ok8. The liver was swollen and increased in echogenicity.  Texture is finer. The gallbladder was normal in size with a distinct mucocele.9. The Colon - ok10. Intestines - okLaboratory: Right lateral radiograph of the chest showed a Vetebral Left Atrial Size (VLAS) is 2.3 (VLAS < 2.3 - likely stage B1 - recheck 6-12 months, VLAS 2.3-2.9 likely stage B2, VLAS > 3.0 consistent with stage B2) Laboratory: Ultrasound scan of the right side of the heart showed LA:AO (Left atrial diameter to Aorta diameter ratio) is around 1.7Assessment: Hardly any change compared with 3 months ago.  Borderline stage B2 heart disease - might benefit from pimebedan medication , but perhaps not at this stage
