Admitted: 9am d/o CONFIRMED Eye enucleation- right eye Estimate on file E2350 pab, ivf, nails o gave stress free last night and this morning + thyroid and eye meds 7:30am 250mg traz PO O gave 1200mg gaba last night and this morning, little effect Had 1/2 insulin dose this AM but no food PAB - BG 2.4 T4>129 Remainder biochem WNL Start IVFT 1 bag of saline with 50IU insulin added set at 0.5ml/kg/hr + 5% glucose solution with 40mmol KCL added at 6ml/kg/hr Procedure: Eye enucleation IOP >99 R eye non visual Anaesthetic Procedure Predmed: Medetomidine 17mcg/kg & Methadone 0.3mg/kg SC Induction: Alfaxan CD drawn up @ 2mg/kg and given IV to effect Maintenance: Isoflurane via endotracheal tube Recovery: Smooth, see anaesthetic record for more details Retrobulbar nerve block instilled 5ml lignocaine Surgery: Transpalpebral approach performed Minimal haemmorhage, controlled with cautery Subcutaneous tissue closed with 3-0 filaxyn Skin closed with 4-0 dermalon top ends left long for removal BG monitored throughout day Ears checked -- nil d/c, suspect was rubbing due to eye pain Plan: - sutures out in 14 days - ecollar & cold compress next 48hrs - carprofen 100mg BID for 7d - current T4 dose is 5 tablets daily, reduce to 3 tablets daily (1.5 mornign & night) and recheck T4 levels in 2-4 weeks - consider testing urine dipsticks daily, may require dose drop :: Added by Vanessa Wong, Tuesday 3rd February 2026 11:21 AM :: LMOM to call if any concerns.
