<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>Verified: Yes</p> <p>HISTORY<br>Presenting for: Hugo presented for a broken nail on the right back paw (digit 2) that occurred a yesterday morning ago <br>Historical Conditions: Previous visits mainly for grass seeds</p> <p>EXAM<br>Mentation: BAR<br>Wt: 18 kg<br>Behavioural: Very vocal and distressed when foot was held up, screaming during examination<br>Integumentary: Broken nail on right rear paw -digit 2- nail snapped cleanly at the base and was completely removed without need for surgery or sedation</p> <p>ASSESSMENT<br>Broken nail - completely avulsed at base, successfully removed in its entirety</p> <p>PLAN<br>New Meds/Tx:<br>- Antibiotics (Amoxyclav) one tablet BID for 5 days to prevent infection<br>- Meloxicam for pain management (18kg dose using kilogram syringe)<br>- Cone collar to remain on for comfort and protection<br>- Bandage applied for stabilisation and comfort</p> <p>Additional Discussion:<br>- Nail will regrow over approximately one month<br>- Bandage can be removed in 3-4 days once patient adjusts to comfort level<br>- Continue cone collar for a few more days after bandage removal<br>- Keep bandage dry and clean<br>- New nail growth expected but will take several weeks to become visible</p> <p>Client Comms:<br>- Call if any concerns arise during healing<br>- Monitor for bleeding (none noted during procedure)<br>- Bandage provides stabilisation and should improve comfort significantly</p></html>
