05/03/2026 Reason: has rash around his testicles 

History: Mrs presenting. Main concern is testicles, not licking but seems to have a skin rash and no hair left around groin. Paws are also itchy and smelly. O has since the last visit not changed the diet or done any medicated baths as recommended... No other concerns. O thinks Cooper is allergic to all kibble, and can only tolerate wet food??

CLINICAL EXAM:
Temp: not taken
HR: 96bpm
RR: panting
Chest sounds clear.
Demeanour: BAR, happy staffy
Oral exam: MM pink, crt<2secs.  Dental grade:  1/4
Abdo palpation: NAD.
LN: NAD
Eyes: NAD
Ears: NAD
BCS:  5/9   Weight assessment: Good body condition.

Other findings:
- ID skin on all paws have moderate erythema and mild-moderate thick discharge.
- Testicles themselves look normal but surrounding skin is warm and moist. No broken skin noted. 

- Cytology from paws and groin area both show cocci +++, no rods or yeast noted today

Problem list: pedal & groin pruritus

Assessment: I think Cooper has poorly managed allergies, we need better compliance- o agrees. Discussed allergies, symptoms, dx, tx etc. 

Treatment: 
- Cytopoint ~2mg/kg to break pruritic cycle. 40mg SC tolerated well. 
- Cephalexin 600mg SID x14d, recheck booked before finishing tx
- Malaseb! SID x3-5d then weekly as maintenance on paws and groin, printed label for o
- Consider elimination diet with hydrolyzed protein, kibble or wet food is fine as long as strictly hypoallergenic

Plan: tx as above, recheck booked.
