ANIMAL PRESCRIPTION This prescription is for ANIMAL USE ONLY PRESCRIPTION MEDICINE Rx: Chlorsig Eye Drops 0.5% 10mL - Chloramphenicol Mitte: 1 bottle Sig: Apply 1 drop to right eye 4-6 times daily Number of repeat supplies: 2 Precautions: nil Hx; here for rechck of eyes O has still noticed right eye is squinting still O has been apply chlorsig eye drops to the right eye Has been appllying pred forte drops to left eye PE: Left eye- normal RIght eye- Fl positive- pin prick - exact same as previously , conjunctival hyperemia Assessment: No change in pin prick ulcer - seems like a corneal scar maybe present Tried to do debridement of ulcer but wont tolerate Started on ocuflox and chlrosig six times daily Start on meloxicam RV in 2 weeks
