<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>History: <br>Given a pigs ear on Sunday (4 days ago) noted that he choked and regurgitated a large piece which he promptly ate again. <br>Has since been weird with food and would regurgitate up when eating and would have abnormal swallowing when drinking - increased effort to swallow <br><br>O were away the last 2-3 days and Gus was staying with O parents. they reported that Gus was the same - regurgitating and having difficulty swallowing. but still able to finish food. <br><br>no D+ noted. still BAR, urinating fine. <br><br>Examination: <br>BAR - good dog. <br><br>Dental - clean teeth, no visible swelling in oral cavity <br>mild discomfort and swallow when palping around cranial 1/3 of neck/oesophagus. <br>Given a treat and significatn effort and neck stretching to swallow treat <br><br>CV/Resp: WNL <br>Abdo: mild discomfort on cranial abdominal palpation, no palpable masses <br><br>rest of PE unremarkable <br><br>Assessment:<br>Ddx - pancreatitis vs partial oesophageal obstruction vs soft tissue injury <br><br>Plan: <br>given est of $1300- 3000 for bloods + admit + radiographs +/- endoscope under GA. <br>handover to CJ/SW <br><br>Procedure: CJ<br><br>Sedated: Butorphanol 0.3mg/kg, Medetomidine 0.01mg/kg<br>Radiographs performed, no abnormalites noted thorax, soft tissue swelling noted around larynx<br>Laryngascope exam showed swelling dorsal - right lateral aspect of larynx, just in front of it, putting pressure on r vocal folds. <br>General anaesthetic performed for intubation, area proped and suspected trauma/abscess, can not rule out migrating foreign body.<br><br>Plan:<br>Advised O trauma vs migrating foreign body, less likely neoplasia given age/presentation<br>Prescribed Noroclav 250mg 1.5 tablets BID 7 days<br>Meloxicam 0.1mg/kg SID PO 7 days<br><br>REview after 7 days,<br>Recommend repeat GA for re assessment OR prompt referral to CT in Hobart to assess for foreign body and possible surgerical planning - surgery may be required.</p></html>
