03/03/2026 Reason/concerns: P has been licking areas on R lateral elbow and also between toes LF paw On gabapentin 400mg BID, helps for OA pain Preventative Healthcare: Vaccination and parasite (flea, intestinal worms, heartworm). Examine: Mentation: BAR BCS: 2/5 MM: Pink CRT: 1-2 seconds HR: 92 beats/min RR: panting breaths/min Temperature: not taken today Ocular: No abnormalities noted on cursory examination of both eyes. Oral/Dental stage: Some gingivitis and calculus observed. Ears: No abnormalities noted on cursory examination of both ears. Cardiovascular: No audible murmur or arrhythmia and no difficulty hearing the heart. HR = PR and no evidence of hypoperfusion. Respiratory: Respiratory rate and effort considered normal for the patient. Gastrointestinal: No pain on abdominal palpation. No masses or abnormalities palpated. Integumentary: Musculoskeletal: Normal gait and strength noted. No evidence of lameness. Urogenital: No significant abnormalities noted during routine physical examination. Neurological: No evidence of neurological impairment or altered mentation. Detailed neurological examination not performed today. Lymph nodes: Within normal limits for mandibular, prescapular and popliteal examination. Reason: General checkup History: Not geetting up as stiffly, carprofen working really well Still waking up quite early in the morning Ears look significantly better - O really with happy with how these have responded to the ear drops Does seem to Ongoing medical conditions - Previous TPLO Current medications - have given her dermotic UTD deworming/ectoparasite – Y Examine: Mentation: BAR, friendly BCS: 6/9 MM: pink/moist CRT: <2s HR: 82 RR: pant 2+ Temp: Not performed PLN: Submandibular, popliteal and prescap wnl Eyes: OU WNL, no squinting or discharge, PLR symmetric Oral: mod periodontal grade, G2/C2 Ears: - both ears significantly improved, mild discharge, minimal erythema Cardio: reg no murmur Resp: nsf, no increase in effort or sounds GIT: no pain on palpation, no obvious mass effect, no fluid wave Integ: small lump now scarred and no longer palpable - green staining still present Cytology: Scant malassezia in both ears (1 per hpf) Problem List: 1/ Bilateral severe otitis externa - scant malassezia, cont 7d then stop 2/ Cognitive decline (?) 3/ Pain in joints and limbs - Good response to carprofen, continue 4/ Mass on ventral chest - resolved, no longer present 5/ Green staining fur on rump --> ddx bacterial infection vs artefact/contamination Symptomatic tx: Finished off ear drops (1 more week) Ketoconazole 2% / enrofloxacin 1.5% / dexamethasone 0.1% ear drops Carprofen 50mg BID PRN - script dispensed Add in Gabapentin 400mg BID Add in paracetamol for painful days IF needed Follow-up: Revisit in 1-2 months to repeat CBC/biochem for monitoring
