02/03/2026 Diagnosis: Mediastinal lymphoma; high grade, hypercalcaemia at diagnosis; T cell lymphoma; pre-treated with L-spar and dex/ pred, CHOP (2 cycles)/ LOPP protocol (3 cycles); relapsed at end of 3rd LOPP cycles; changed to Doxorubicin #4/ cyclophosphamide?

History: Banjo's appetite improved a day following his last visit for his nadir.His has been having soft stools infrequently for a while, but he is otherwise well

Physical Examination findings: Bright, alert and responsive.
Heart&Lungs: no murmur or arrhythmia on auscultation, normal respiratory effort and bronchovesicular sounds bilaterally
Abdomen: soft and comfortable
Rectal: soft stools on glove + some debris, normal intestinal lining no internal lymphadenopathy
LN: all peripheral lymph nodes palpate normally
MSK; ambulatory x4, normal gait
Integ:one pigmented, raised and well demarcated mass on the left thorax (static)
Mm pink, crt < 2 secs, HR 112, RR pant, Temp 38.5, Wt: 9.85

Laboratory results: CBC performed in house with manual blood film assessment.
Hct:0.357 
WCC:4.83 
Neuts:3.78
Platelets:19

Manual differential count:  Neutrophils 91%, Lymphocytes 3%, Monocytes 5%, Eosinophils 1%.
Platelets: clumped and adequate
Morphology: Normal 

In- house ionised calcium (i-stat): 1.28mmol/L was 0.99 mmol/L

Treatment: xmg doxorubicin (in between dose 1mg/kg and 30mg/m2) was administered IV via a 22g catheter in the x cephalic vein using the closed injection Equashield system.
1mg/kg maropitant administered IV immediately prior to chemotherapy
Anti-nausea as required
Pro-kolin as required for diarrhoea. 
Continue denamarin
