Reason: Transfer Leg FractureConsultation time:20:00  History:Presenting problem:  NWB RFL and crying after jumping out of car this afternoon Seen at PCP; susp fracture radius/ulnar - discussed case with Alistair who has OK'd sx if required on Monday (est 5-6K) Given methadone & meloxicam at PCP 1700 Previously healthy dog Not on any medications   Examine:  Demeanour - BAR T:   37.5    HR: 72   RR:  pant   mm:  pink moist   CRT: <2 Weight:  2.09kg Cardiovascular - no murmur/arrythmia Respiratory - lung fields clear bilaterally Gastrointestinal - NADUrogenital -  NAD Musculoskeletal - NWB RF - painful distal R/U and appears broken Integument - NADNeuro - NAD Eyes - NAD Lymph nodes - NAD      Imaging:Radiography- Region: FLs Radiographic findings: Tranverse radial ulnar fracture Procedures:- Modified Robert-Jones bandage placed on the right forelimb, tongue depressor used as a splint on the caudal surface of the antebrachium Problem list:Tranverse radial ulnar fracture Assessment:Discussed admit for sedation, analgesia and radiographs tonight Place Fentanyl patch and once effective- discharge and sx booked for Monday with Alisdair Treatment: - Methadone 0.2mg/kg IV q4h - Fentanyl 12mcg/hr patch placed (right metatarsal region) @ 2145 Plan:Sx with Alisdair Monday Fentanyl patch on board, oral meloxicam & strict rest--------- Hospital vet overnight update - AG ------------Update:- Stormy has eaten 100% of offered meals overnight- mRJ bandage remains in place - Adequate analgesia on Methadone q4hTreatment: as above- To commence Meloxicam PO as dispensed by PCV @ 1700 14/3 and discharge with Fentanyl patch in situ Plan:- Fracture fixation on Monday with AR    Vital Signs
