<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>History:<br>- Presented for facial swelling on the left side, noticed by owner's husband when he returned home. Owner was brushing teeth earlier in the day and did not notice the swelling at that time.<br>- Teeth are not brushed regularly. Has had significant dental work in the past with multiple teeth already missing. Has a large underbite which is less noticeable now that the front teeth are gone. Breath has a noticeable odour.<br>- Energy levels have been normal. Was waiting for dinner prior to presentation and no other symptoms were observed.<br>- Pre-existing dental disease. Clicky knees diagnosed by previous veterinarian in Queensland. Currently on 4cyte with dinner each night for joint support. Has been relatively healthy throughout life. Blood tests have been performed in the past as a senior dog, though exact timing is uncertain and may have been over 12 months ago. No other known medical issues. Not currently on any other medications.<br><br>Physical Exam Findings:<br>- Weight: 5.98 kg<br>- Behaviour: Not cooperative during oral examination, was painful when mouth was examined.<br><br>Eyes:<br>- Swelling present on the left side around the eye and cheek area, approx. 5cm in diameter<br><br>Oral Cavity:<br>- Pre-existing dental disease present. Significant underbite noted. Multiple teeth missing from previous dental procedures. Oral examination limited due to patient discomfort.<br><br>Cardiovascular:<br>- Heart sounds normal on auscultation.<br><br>Assessment:<br>- Problem list:<br>1. Suspected tooth root abscess secondary to pre-existing dental disease<br><br>Plan:<br>- Clindamycin one tablet once daily for 14 days for dental infection<br>- Meloxicam liquid anti-inflammatory for pain relief, to be given once daily<br>- Dental procedure recommended within the next two weeks for tooth extraction as definitive treatment<br>- Blood tests recommended prior to commencing anti-inflammatory medication given age and to assess kidney function; owner elected to proceed with medication with awareness of potential risks if kidney impairment is present<br>- If vomiting or diarrhoea occurs, cease meloxicam and contact the clinic<br>- Follow-up with local veterinarian once established<br><br>Communications:<br>- Discussed that the most likely cause of the facial swelling is a tooth root abscess secondary to pre-existing dental disease. Explained that antibiotics will help reduce the infection and swelling but will not eliminate the abscess completely as antibiotics require blood supply to work and will not penetrate the central pus-filled area. The only definitive treatment is removal of the affected tooth. Discussed the risks of anti-inflammatory medication in older dogs and the recommendation for pre-anaesthetic blood tests given age and that bloods have not been performed in the last 12 months. Owner elected to proceed with anti-inflammatory medication with awareness of potential risks. Discussed local veterinary options including Jerrabomberra, Googong, and Queanbeyan clinics. Advised that records can be transferred to the new regular veterinarian once established.</p></html>
