Reason: NPC - Happy Tail, Split At The End (MM)Appointment Notes: Overdue remindersHistory:Presenting for happy tail - flicked tail too hard at wall and started bleeding, has happened before in the past. EDUD normal. No vomiting or diarrhoea. No lethargy.Examine:Mentation: BAR Demeanour: Friendly, food motivated HR: 102, no murmurs or arrhythmias RR: 20, lung fields clear BCS: 5/9 Eyes: NAD Ears: NAD Dental score: Grade 1/4, very mild plaque buildup, no gingivitis  MM/CRT : Pink, moist, CRT <2sLN: NAD Skin/Coat: Small open wound on distal tail, some chronic changes (darkened/ black skin) at distal tail surrounding the wound. Fleas seen: NAbdomen:  Comfortable                       Limbs/Gait: Ambulating well, weight-bearing on all four limbs. Assessment:Happy tail - mild, not actively bleeding Treatment: Wrapped tail in small amount of soffban, placed a 60ml syringe at the distal end of tail to protect it. Secured with elastoplast. Amoxyclav 500mg - 1 tablet PO BID Plan: Sent O home with elastoplast roll to re wrap the bandage if needed. Or can bring her in to re wrap if it falls off Recommended e-collar if chewing at tail.Recommend recheck in a week. Do vaccines at the same time.
