 History:- Dachshund, just turned 1 year old, intact female- Puppy farm origin, gets stressed easily- Lives on acreage, indoor dog with supervised outdoor access- Scavenger - eats possum poo, underwear, chews various objects- Diarrhoea started Wednesday ~1pm, nearly 48 hours ago- Initial solid stool followed by liquid diarrhoea- Blood noted in diarrhoea- Vomiting started yesterday - projectile after eating Hungry Jack's chip- Multiple bouts of diarrhoea overnight, last episode 4:30am- Drinking well, maintaining hydration- Appetite present - ate boiled chicken and rice yesterday- Given half pouch kangaroo Lyka (sensitive food) morning of onset - more than usual portion- Previous episode of vomiting/diarrhoea when very young- Just finished first heat ~2 weeks ago- No mating - only around desexed male dogExamination:Demeanour/Temperament: Super friendly, normal for PtEyes: NADEars: NADCardiovascular: NADRespiratory: NADAbdominal: Thickened intestines, passing excessive flatusRectal: NADIntegument: NADLymph nodes: NADMusculoskeletal: NADNeurological: NADUrogenital: Nipples puffy post-oestrusOral: Mucous membranes pink, well hydratedAssessment:- Acute gastroenteritis - suspected food poisoning (Salmonella/E. coli)- Hepatic involvement ?? - bile present in diarrhoea- Digested blood in faeces consistent with GI inflammation- Foreign body to be ruled out given scavenging behaviourDiagnosis:- Suspected food poisoning pending XRs and bloodsTreatment:- 24-36 hour fast, water only- Anti-emetic injection (36 hour duration)- Gut antibiotics for Salmonella- Hospitalisation for abdominal radiographs and bloods (liver/kidney function)Plan:Admit for abdominal radiographs to rule out foreign body obstruction. Blood tests to assess liver and kidney function. Results expected early to mid-afternoon. O to be contacted with findings. Desex recommended in 3 months (June) when oestrogen levels at lowest for optimal clotting.Client Communication:Discussed suspected food poisoning as most likely cause given history of scavenging and possum poo consumption. Explained that foreign bodies cannot always be palpated on examination. O opted for hospitalisation and diagnostics given pet insurance coverage.
