Previous medical/surgery history:
- Dog reactive.
- Osteoarthritis (OA).
- 19/01/26: Monthly beransa and pentosan, lump noted RHL 
- 17/11/25: Last Urinalysis in house - dipstick -Collection method: voided , pH: 6, USG:  1.011, DQ NAD with one clump of epithelial cells , WP nad
- 19/12/25: Suspected Renal Disease (or other systemic disease impacting renal concentrating ability). Undiagnosed as owners declined bloods
- 04/07/2023: KC vaccination.
- 22/12/2023: Emesis induced for chocolate ingestion. Pentosan started for OA.
- 01/09/2021: Surgical removal of 2x trichoepitheliomas (tail base, L) cranial scapula) and 2x masses on L) femur (not sent for histopathology). Dental procedure performed.
- Pre-anaesthetic bloods on 01/09/2021 showed mild non-regenerative anaemia (sample taken post-sedation).
- Post-operative wound complications managed with staples and glue.

Current medications and doses given:
Pentosan 1.65mLs
Beransa 30mg 
meloxicam 1.5mLs - tapering to lowest effective dose

-cocnern re euthenasia and QOL

- problems raising, walking and increased falling
- Increasing unable to instability- but other days will move ok

Assessment 

concern re renal scald when unable ot move
 QOL decision.

Plan
 have pre-booked HV euthanasia and private cremation
