Reason: VomitingAppointment Notes: BNHistory: Vomited this am, pale yellow fluid, does chew random stuff in the garden, had more V+ in the early week, possibly eaten some mushrooms growing in the garden. No D+, did poo in the shower twice when sick - but they were formed. Frequent recurrent 5am vomits, over the last few weeks, is fine during the day. Energy levels and play all normalDiet: hills diet kibble (unsure which) been on for a long time. Examine: 1. BCS:     2.5/52. Integument: nad3. Musculoskeletal: nad 4. Teeth: minimal calculus no gingivitis5. Eyes: nad 6. Ears: nad 7. Lymph Nodes: nad 8. Cardiovascular/Circulatory:   nad  - no murmur noted9. Respiratory/Pulmonary:  nad 10. Uro-genital: nad 11. Gastrointestinal: nad - abdo palp comfortable, no nausea, no pain, no mass effect, eager for treats in consult->11a Anal Sacs: not examined 12. Behaviour: BAR, 13.
