Reason: SeizurePresented and seen by Dr Jasmine Annan  @ 1am 13/03/2026Regular Vet: SAH KellyvilleHistory: Presented post seizure- O said lasted about 5 minutes- lost consciousness, paddling, vocalising loudly. Owner said came to was growling and very disorientated and stressed. Has been stressed and panting heavily in car- not his normal self, trying to climb up windows.First seizure, but has been having behavioural changes and decline at home. Also has tracheal collapse.Said has been to vet recently- cognitive decline. Has been deteriorating at home- owner feels it may be time to PTS tonight.Objective: Demeanour: Post Ictal, Disorientated, VocalisingTemperature: 39.2C.Cardiovascular: 200, hard to auscultate over respiratory noise, FP synchronous and adequate.Respiratory: Panting, increased effort, crackly bronchovesicular soundsOral exam: CRT 1s, mms injected, marked dental disease.Abdomen: Soft, doughy abdomen, no pain elicited, no mass effect.Peripheral LNs: No enlargement.Eyes/Ears: nuclear sclerosis, blepharospasmIntegument: patchy alopecia whole body- sparse coatMusculoskeletal: non ambulatory, hard to asses as flailing- very stressedGait: Non ambulatory, knuckling forelimbsNeurological: Disorientated, CN-- nil menace, PLR cannot assess as sclerotic, palpebral reduced, CP reducedRectal exam: WNL.Urogenital: WNL.BCS: 5/9.Assessment: Seizure activity- post ictalDdx: Intracranial (neoplasia, CVA, inflammatory, infectious, degenerative, epilepsy) Vs Extracranial (metabolic, organ dysfunction, toxin, neoplasia, other)Client Communication:Discussed with owner- post ictal and very distressed- inappropriate mentation. Have placed IV catheter and has been given Diazepam to settle.Discussed potential causes with owner- given age concern for intracranial causes- neoplasia, CVA etc.Advised options1. Blood work for extracranial causes, Seizure watch and monitoring overnight, medications for seizure cotnrol. Discussed refractory seizures and may need CRI etc. Adv intracranial causes need MRI for diagnosis.2. Monitor at home-- concern as currently not recovered and may decline or re-seizure3. Humane Euthanasia.Given current state and decline- owner elected to euthansie tonight, feels he has been declining rapidly and does not want him to suffer further.Treatment:Butorphanol 0.2mg/kg IM @ 1amIVC placed in LFL cephalic- Diazepam 1mg/kg IVLethobarb 10ml IV Death confirmed by cardiac auscultation.Plan: Body on hold whilst O decided on vessel options    Vital Signs    Weight: 6.7;
