History: Meningitis management. Fentanyl patch removed yesterday and Zambi reported to be doing well. This morning wasn't interested food, flat/lethargic. Spoke to webvet. Current meds: Prednisolone, Paracetamol (this morning), Clindamycin Examination: HR: 110RR: pantTemp: 38.4MM PM<2sMentation: QARHydrated Cardioresp: nadGI: nadUrogenital: nadSkin: nadLymph nodes: no enlargement Musculoskeletal: ambx4, no limb or spinal pain. Stiff neck, uncomfortable with ventral flexion. Neuro: nsf@2:30pm patient patient went for walk to toilet, urinated, more bright than first admitted@3:00pm offered GI food in hospital Assessment: Mild lethargy and neck discomfort. No fever, hydrated and able to ambulate normally. Trial placing another fentanyl patch for pain relief. Treatment: Consent has been given by the client to proceed with this treatment - YesFentanyl patch placed on LHL @3:20pmPlan:Continue with current medications with addition of fentanyl. O to closely monitor. Reassess on Monday. Webvet and/or emergency if concerns over the weekend.O communication:Spoke to owner at 3pm to update.
