HISTORY-
Been on 20mg frudix PO BID currently and going well, incontinence has improved with moving doses earlier, appetite improved, coughing maybe 3-4 times per day and RRR around 30bpm each night. 
Diet: chicken drumsticks + veggies + rice etc
F/T/W/HW: UTD

cardisure 2.5mg PO BID
sotalol 80mg 1/2 am 1/4 pm
codeine 30mg 1/2 BID

EXAM-
Demeanour: BAR
BCS  2.5/5 gained some weight
HR:  140bpm
RR:   pant
T:    38.3
CRT: 1sec    
MM: pink & moist
CHEST: Grade 4/6 systolic murmur, pulses S&S, lung sounds clear no crackles, 1 VPC audible in 60s
EYES: NAD
EARS: NAD
NOSE: NAD
ORAL EXAM:  NE
SKIN:  NAD
ABDOMINAL PALPATION: NAD
MUSCULOSKELETAL SYSTEM: NAD

LAB:
Comp - mild to moderate azotemia + moderate hypokalaemia
new since starting furosemide

ASSESSMENT:
stable heart dz
worsening renal dz

Tx:
reduce furosemide to 10mg (1/2 tab) PO BID
script renewed for other meds

PLAN:
Recheck bloods in 1mth to reassess on new dose - call in a few days to let us know how she's fairing with her coughing etc. 
Discussed K supplementation - banana 1/2 in her diet, to look into more natural supplements rather than another pill.
