Owner reports allergies flaring up again with new sore on head today. Vomited grass yesterday or day before. Hasn't been himself. Eating but not drinking much water. No diarrhoea seen. Wouldn't get up this morning, had to be picked up and taken to food. Back legs shaky this morning, indecisive where to go to toilet. Belly very warm this morning. Occasional sneezing. No coughing. No crying. Occasional scratching, saw some yesterday. Had Cytopoint injection 2 weeks ago.Current medications: Malaseb shampoo twice weekly, Simparica Trio (flea/tick prevention)Examination:Vitals: - Temperature: Normal - Weight: 37.4 kgPhysical Exam: - General Appearance: Not himself, miserable - Integument: Infected skin on head, severe interdigital dermatitis on front paws, calluses on elbows, interdigital dermatitis on left foreleg, hair loss and scabs on caudal lateral thighs bilaterally, papules on ventral aspects, malodorous - Lymph: Submandibular lymph nodes enlargedDiagnostic Results: Blood test taken to check liver, kidney, pancreas functionASSESSMENT:Secondary bacterial skin infectionAllergiesPLAN:Diagnostic/Treatment Plan: Antibiotics - one tablet twice daily starting tomorrow morning. Malaseb shampoo every second day when skin looks bad, less often when improved. Continue Cytopoint as needed for allergies.Recommendations: Blood test results to be called tonight. Follow-up call Friday to check progress. Call back if skin doesn't completely resolve after antibiotics for more medication. Shampoo is main treatment, antibiotics are adjunctive.
