Previous medical/surgery history: 23/02/2026: Fibroadnexal hamartoma with marked pyogranulomatous and lymphoplasmacytic inflammation excised from RH medial aspect with 2mm margins. 18/07/2025: Pancreatitis with elevated liver and pancreatic enzymes. 13/08/2021: Grade 1 soft tissue sarcoma removed from right elbow with incomplete margins. 04/2018: Spayed. 12/03/2026 Consultation nurse: AH REASON: Recheck and bandage change. HISTORY: The bandage was noted to be slightly wet. Mild dermatitis was observed just distal to the wound. The wound is contracting and has a healthy granulation bed. Previous medical/surgery history: 23/02/2026: Fibroadnexal hamartoma with marked pyogranulomatous and lymphoplasmacytic inflammation excised from RH medial aspect with 2mm margins. 18/07/2025: Pancreatitis with elevated liver and pancreatic enzymes. 13/08/2021: Grade 1 soft tissue sarcoma removed from right elbow with incomplete margins. 04/2018: Spayed. EXAMINATION: Integument - Mild dermatitis noted distal to the wound. The wound is contracting with a healthy granulation bed. TREATMENT: - Bandage changed. The wound was cleaned with saline, manuka honey was applied, and it was covered with melolin, velban, and a conforming bandage twice. Cotton was placed between the toes. The bandage was finished with Tensoplast with stirrups and bitter wrap - Antibiotics prescribed: 3/4 tablet BID 8 days, starting today. - Firocoxib 227 mg:1/2 tablet SID with food 8 days. Advised to stop if vomiting or diarrhoea occurs. - Advised to keep the bandage clean and dry and to use a bandage cover. PLAN: - Recheck and bandage change scheduled for Tuesday, 17/03/2026, in 5 days to monitor progress.

Bandage has not been bothered but did come off once.

EXAMINATION:
Demeanour- BAR pink moist mm crt<2s
Integument - Wound on RH medial aspect. Nice granulation bed present. Mild redness and inflammation around the wound margins. No evidence of licking. Bandage removed. Distal toes non-swollen. Wound is generally clean and dry.
ASSESSMENT:
1. Post-operative wound management - Healing as expected with mild inflammation. Histopathology confirmed benign fibroadnexal hamartoma with complete margins.
TREATMENT
1. Post-operative wound management
- Wound cleaned with saline. Manuka honey, melanin bandage, and layered Velband/conforming bandages applied. A no-chew bandage was placed as the final layer. Distal toes left exposed to monitor for swelling.
- Continue firocoxib 227mg 1/2 SID PO for 10 days.
- Continue anxiety medication to encourage rest.
- Recheck bandage change in 4-6 days.
- Advised strict rest. No swimming or getting the bandage wet. If bandage becomes wet, it should be removed, the wound cleaned with warm salty water BID, and left open to air.
- Vaccinations are overdue.
PLAN:
- Recheck for bandage change in 5 days.
- Continue current medications.
- Strict rest advised.

Previous medical/surgical concerns: 23/2/26 lump r/o RH medial aspect - DIAGNOSIS Fibroadnexal hamartoma with marked pyogranulomatous and lymphoplasmacytic inflammation (excised) with 2mm margins around. 18/07/2025: Pancreatitis with elevated liver and pancreatic enzymes. 13/08/2021: Grade 1 soft tissue sarcoma removed from right elbow with incomplete margins. Assessment:Wound granulating well. Contracting Treatment: cleaned wound with saline. bandage change and replaced with melolin with manuka honey Velband Conforming bandage Vetwrap, tensoplast then finished with vetwrap again. Left toes out today and tensoplast up to hock. Gave Os some material incase it falls off again. Plan: recall, recheck revisit repeat bandage change in 3 days then drop down to weekly. May just bandage to below hock in subsequent changes if it won't stay on.
