SUBJECTIVE:sensor placed 6 days ago finished clav last night bumping into things - anxious about this, sticking close to owner going ok giving the injections 13IU toujeo SID PM in past 5 days have 2 days of nadir around 15, one naidr 18 and 3 days glucose stayed above 20 OBJECTIVE:quiet, alert MM Pink chest NAD bilateral cataracts blind mild scleral hyperaemia / chemosis ASSESSMENT:Diabetes mellitus PLAN:acular BID both eyes -may be able to reduce to SID in future check eye pressure at next appt increase insulin from 13IU to 15IU SID from tonight CLIENT COMMUNICATION: will talk on phone over next few days recheck 1 week to discuss, remove and likely replace sensor, check eye pressure, O to bring in urine sample from that morning if able to chk UTI resolved

O called as concerned that glucose reading HIGH, even after insulin dose. Ate dinner but seems lethargic and not wanting to get up much Spoke to NB who recco monitoring overnight. If still all readings high tomorrow call clinic and we may increase dose
