12/03/2026 Presenting Complaint: 14yo Devon Rex for vaccination, annual health check, blood test. Owner reports increased thirst. Owner reports fall from stairs a few days ago. Owner reports weight loss. Hx: BIB Tara Fell due around time of another pet's event. Currently 5 months overdue. History of being overweight, previously on weight loss plan. Was over 5kg, now 4.2kg. Weight loss of ~1kg since September (4-5 months ago). Was 5.13kg at last visit 4 months ago. Fall from top third of stairs to bottom fourth a few days ago. Landed on side, not on feet. Owner has not noticed any lameness but has been feeding at a lower level to prevent jumping. No signs of pain now.. Mostly an indoor cat. no c/s/d/v, eddu all ok - poss icnreased thirst but coincided with hot weather Diet: Hills Gastrointestinal Biome. Gets some kangaroo snack at night. Sometimes eats shredded chicken and tuna, licks jelly/water off and leaves the meat. Gets Greenies and dental crunchies. Aldi tinned food tuna, shredded chicken Medication: nil Parasite and Vaccination Preventive: Vaccination overdue by 5 months. PEx: Temperament: BAR, escaping. BCS: 5/9 MM: pink and moist, CRT: 1-2 sec Mouth: g1-2 Dental tartar building up. eyes: NAD ears: NAD nose: NAD Cardiovascular: HR: 200 PR Murmur: No Respiratory: RR =50 lung sounds are normal Lymph nodes: NAD Abdominal Palpation: NAD. Non-painful on palpation. Skin/Coat: NAD Musculoskeletal: No lameness or pain detected on manipulation despite recent fall. Genitalia: Rectal temperature: 38.7C Other Issues: Weighed 4.2kg. Previous weight was 5.13kg in September (4 months ago). Laboratory results: weight loss geriatric cat Blood and urine samples collected. Bloods collected for full profile including biochemistry and haematology to check kidney function, thyroid etc. Small sample volume for haematology. Urine sample collected via cystocentesis. Assessment: 14yo cat for wellness check. Significant weight loss (~1kg in 4 months). Polydipsia reported by owner. Recent trauma (fall). Dental g1-2 tartar. Primary differentials for weight loss and polydipsia in an older cat include hyperthyroidism, chronic kidney disease, diabetes mellitus. Blood tests will help differentiate. Tx: Tricat 2509302 03/28 sc Plan: Blood and urine sent to external lab for analysis. Results expected tomorrow or Monday. Plan dependent on blood results. May include management changes such as starting a kidney diet. Continue dental diet/Greenies. Advised owner to monitor for any signs of pain/lameness from fall. Recheck to be scheduled based on lab results. Consult Summary: Annual health check and vaccination for 14yo cat. Significant recent weight loss (~1kg/4 months) and increased thirst. F3 vaccination administered. Blood and urine samples collected for full workup. Awaiting lab results to guide further management. Owner advised re: vaccination booster protocol vs actual risk.
