<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>H<br>- lethargic  at home<br>- saw a ceritfied dog masseus yesterday - massage person noted thoracolumbar pain and left patellar pain<br>- still eating less at home and seems flat <br>- still vocalising when picked up (O picks him up under the arms)<br>- drinking at home<br>- no bunny hoping gait, vocalisation or lameness while exercising<br><br>E<br>- ears, eyes, nose: NAD<br>- MSK: no pain on cervicothoracic palpation or neck movement (full ROM), no vocalising on any other spinal palpation. No patella luxation noted on concious examination bilaterally or hip movement./ Ambulating well in consult. <br>- abdo: pain on palpation of left cranial abdo (but also is a very sensitive dog to touch)<br>- MM pink + moist, CRT &lt;2s<br>- xray (left lateral and DV) - no radiographic changes to any of the spinal column, ribs, pelvis, thoracic or pelvic limbs, and hips. Abdominal organs WIL. <br><br>A<br>- no radiographic evidence of MSK changes, ddx: thoracolumbar soft tissue injury +/- early grade patellar luxation<br>- pain on abdo palpation: intraabdominal pain (pancreatitis, gastroenteritis, hepatic disease) vs touch sensitive (seems to growl in different areas across the last few examinations)<br>- difficult to differentiate if this is thoracolumbar MSK pain vs abdo pain in this dog<br><br>P<br>- showed O xrays and discussed nothing obvious to explain the pain response<br>- added 10mg/kg gabapentin BID for 5 days + meloxicam SID PO 4kg dose for 7 days - if no improvement in demeanour/lethargy/food intake consider running bloods, abdo ultrasound to look for intraabdominal causes of pain<br>- if O is still concerned for MSK pain I recc CT as next option to investigate</p></html>

Recurrent yelping when O picks her up. Has tried picking up from stomach, from chest, will yelp. Trialled meloxicam, seemed to improve then started again. Toddler fell on her 2 weeks ago, possibly injured.Is otherwise doing fine.Exam: BAR, sweet,MSK: no proprioceptive deficits, normal gait, normal stance. Growled when palpating over cervico-thoracic spine but then also growled all the way down. No yelping when picked up in consult. Ribs not painful on palpation. Full ROM in neck. Inconsistent pain response when palpating over back. Sometimes growls sometimes not.DDx soft tissue injury vs IVDD vs behavioural?Plan:Trial meloxicam and rest for another 14d and if no better then xray spine.
