Appointment Reason: 1.2 CONSULT 2, Appointment Notes: Torn toenail - JO Vet MKPresents for torn nailHappened ~ 2 weeks ago on a walk - there was a bit of bleeding that stopped quicklyHasnt been bothered by it but seems to be growing strange - O would like checkedPE: BAR, anxious but cooperative EENT - Bilateral lens opacityLN - External lymph nodes palpably normal - submandibular, prescapular, inguinal, popliteal RESP - Normal bronchovesicular sounds, normal effort CVS - SSFP, no murmurs or arrhythmias auscultated ABDO - Abdominal palpation normal PERINEUM - No discharge or lesions noted in anal gland area. Rectal examination not performed today INTEG - *** Claw of digit 5 LPL split longitudinally and lifted up exposing quick. The quick has dried off and looks healthy - looks like new nail is trying to grow. Clipped the split top part of the nail as short as possible to avoid it getting caught - to remove fully would need sedation as still quite well attached at baseMUSCULO - Normal gait, no lameness identified, full exam not performedUROGEN: No abnormalities on examination of genitals NEURO - Normal mentation. CNs intact, CP normal, pain response present, reflexes normal ASSESSMENT/ABNORMAL FINDINGS: Broken claw digit 5 LPLPLAN;Clipped broken part as short as possibleFine to monitor as not irritating Whiskey - new nail will eventually grow out but can take awhileIf causes any discomfort then sedation for complete removal
