No other concerns. No vomiting, diarrhoea, coughs or gags. Previous treatment included antibiotics (not completed - stopped early when lesion appeared healed), sedation medications, chlorhexidine wash and white cream.</p> <p>Exam:<br>Mentation: BAR<br>Temp: Did not attemptC <br>Cardio: HR 112 = pulses bilaterally symmetrical <br>Resp: RR = 35, some panting<br>Eyes: Currently being seen by specialist. <br>Ears: NAD<br>Teeth: Gde 2/4 <br>Other oral exam: <br>MM/CRT: pink/moist, CRT<2s<br>Skin & Haircoat: Hot spot on back adjacent to previous shaved area from last episode, erythema, moist lesion, some malodour. Previous lesion site not completely healed.<br>Musculoskeletal: <br>Lymph Nodes: WIL <br>Rectal: Did not perform rectal exam<br>Other problems found:</p> <p>Assessment:<br>Hot spot on back adjacent to previous lesion site. Previous lesion site not completely healed.<br><br>Treatment: <br>- Clipped and cleaned affected area<br>- Chlorhexidine wash (O has at home) - dilute 1:10, scrub affected area BID for 10 days<br>- Topical cream (Bacticorten) - apply to affected area BID for 10 days<br>- Antibiotics PO BID for 10 days (full course emphasized)<br>- Gabapentin and Trazodone (O has had before) - PRN for sedation as needed<br>- E-collar dispensed - XL size<br><br>Recommendations & Client Comms: Must complete full 10 day course of antibiotics. Apply E-collar for at least 1 week to prevent licking/fussing. Continue topical treatments BID for 10 days. Use sedation medications as needed if fussing occurs. <br>Revisit 7 days time. </p></html>

Seeing ophthalmology specialist q3mo for ongoing eye condition (cataracts). Hx of skin disease, currently using antibacterial wash. Mild discharge from eye noted today. Area already shaved and cleaned by groomer.Rectal: Did not perform rectal examAssessment:Hot spotPlan:Diagnostics: n/aTreatment: - Antibiotics (amoxyclav) 1.5 tablets PO BID x 7d- Bacticorten cream (owner has at home) - apply to affected area twice a day- Chlorhex wash (owner has at home) - clean affected area one to two times a day- E collar to be worn- Gabapentin 1 tablet + Trazodone 1.25 tablets as needed for sedation (owner has at home but OK to dispense more if requested)Recommendations & Client Comms: Use chlorhex wash, bacticorten and complete full course of antibiotics. Ensure cone remains on and try to separate dogs as much as possible (other dogs will try and lick at wound). Advised can try to use a dressing but will likely just annoy Storm and ideally would like to give the hot spot as much air as possible.
