07/03/2026 Reason: sore nail, vaccination Hx: Yesterday got very excited when O's got home and accidentally ran into their young child, O had to quickly move Reiko out of the way to get to child and he ended up falling into the wall quite hard, hurt his foot. Was initially bleeding from the nail but this stopped quickly. Has been NWB and lame since. Otherwise doing well, O would like to get UTD with vacc whilst he is here. Current medications - none Behaviour- cooperative boy Exam: mm- pink, moist CRT- <2s Eyes- clear, no discharge Ears- no erythema or discharge Mouth/Teeth- grade 1 LN- within normal limits HR- 120 Auscultation- no murmur auscultated Chest- lung fields clear RR- panting Abdo- soft, comfortable Urogenital- nad Neuro- nad Musculoskeletal- intermittently NWB on RFL. Clipped the foot, digit 4 of RFL swollen. Allowing palpation/flexion/extension. Some dried blood at the base of the nail, no appreciable crack in nail and firmly adhered still on palpation today. Skin- nad BCS- 7/9 Assessment: fit for vaccination. Swelling of digit 4 - soft tissue vs traumatic injury to nail vs fracture vs other. Discussed with O findings from examination. Nail is still quite firmly attached today so pulling nail not indicated at this stage. Discussed cannot rule out fracture etc without radiographs. Home on meloxicam today - R/V Tuesday if not improved for radiographs and further assessment of nail of digit 4 under sedation. Discussed weight loss. Vaccine type- Protech Pi2 + Protech BB Injected 1.6ml meloxicam S/C.
