26/02/2026 post op phone call mh o reports recovering well, BAR, eating very keenly and seems comfortable. advised that bb would like urine sent of for c+s and told o cost of this, o would like to speak to bb before doing this to ask why is necessary and querying cost as payed for in house testing yesterday - task left for bb 27/02/2026 Left Vm for O - Culture diff from in house test - tells us what bacter and what ab is effective - Reason for sending was elevated renal values from last test (12 months ago) - could this be secondary to chronic UTI or is this just normal progression of CKD - Florence is not symptomatic (UTI type symptoms) so completely up to O if want to send or not - Removed cost - U in fridge - probably still ok to go for culture if O wishes tonight/tomorrow morning but then should throw out - Apologised for missing her on phone and advised her to book in to chat with me on Tuesday 12-8 at a time that suits her if she would like to discuss this further

History: - Presented for dentistry - Cystitis at start of February GA - Premed: gabapentin and methadone 0.3mg/kg IM - IV catheter placed - Induction: 5mcg/kg domitor IV, alfaxalone to effect. Cuffed size 3.5 endotracheal tube placed - Bradycardia and hypotension treated with fluid boluses 5ml/kg/hr x3 and atropine 0.01mg/kg - heart rate and blood pressure improved - Actively warmed, anaesthesia otherwise stable - Slightly slow recovery - Buprenorphine 0.02mg/kg SC this evening - Pulse: 80-100 (borderline bradycardic) - Blood pressure: low Dental: Oral Cavity: Grade 2-3 tartar. Resorptive lesion 407 - surgically removed, closed with 5-0 PGC suture in simple interrupted pattern. Roots mostly resorbed, removed top section / partial crownectomy. Missing teeth - see dental chart. - Full mouth dental x-rays performed - only resorptive lesion was 407 Diagnostic Results: - RBC 6.02 (mildly low), HCT 0.303 (low end of normal) - WBC normal - SDMA 15 (ref 0-14), creatinine 176 (ref 71-212), urea 14.4 (ref 5.7-12.9) - Urine collected via cystocentesis - in-house analysis pending Assessment: 2. Suspected stage 2 kidney disease 3. Rule out urinary tract infection Plan: - Buprenorphine 0.24ml transmucosally q12h to go home - Post-op check in 3-5 days - Call owner Friday with urine results
