Joy has been doing very well since our last consultation in July last year.  However, she recently began to lick her left front paw and when checked yesterday at Vet HQ had a yeast otitis in both ears.  She is on 50 units of immunotherapy every 10 days, 4mg of Apoquel once daily, nystatin to the vulva as required and Elocon ear drops to both ears every 10 days. A possible trigger is that she has been swimming.On examination the ear canals were not inflamed but a little waxy. The ear drums were intact.  The entrance and inner side of the ear flaps was inflamed and waxy. The front paws were inflamed. The remainder of the skin was clear of infection and inflammationMicroscopic examination from samples from the ears showed yeast in the ear canals but especially on the inside of the ear flap. Occasional yeast were seen on the paws. I will use a short course of cortisone to reduce inflammation and also oral anti yeast capsules to treat the infections. Please give one 5mg prednisolone tablet once daily for 3 days, then half a tablet every 2nd day for a week.Give one 50mg itraconazole capsule once daily with food for three weeks.I would recommend that we avoid swimming for at least 2 months.When you approach the end of the vial of immunotherapy, contact us and we shall order more.  Continue with her other therapies. In one to two years, we may trial stopping the injection’s and see if Joy relapses.
