06/03/2026 Consultation nurse: JW Reason for presentation: unsteady on feet recently History for today: Past 1-2 days not herself, stiff, walking slowly Struggling to jump, can't get into her sleeping pouch EDUD as normal, some difficulty posturing Normally very active still, runs with mum Previous medical/surgical concerns: 04/12/25: Laceration on the right fore paw with an exposed tendon between lateral aspect of the 5th digit and paw pad. Grade 2 heart murmur previously noted on examination. Current medications: Nil Examination: QAR - seems sad BCS 5/9 Eyes clouding Ears clean Oral nad MM pink/moist Hydration good DS gr 1-2 Cardio 136bpm, Gr 3-4 HM bilaterally Resp nad Abdo tense - suspect referred pain given no GI signs MSK: - hunched posture, stiff gait - resistant to lateral flexion LHS - discomfort through entire thoracic spine and cranial lumbar Neuro nad, reflex/proprioception etc normal LN nad Temp Demeanour- Dull - Nervous Eyes & Ears- NAD Nose and Throat- Clear Mouth/teeth/gums/dental grade- gums mm pink crt 1-2s Resp rate- NAD Heart rate- 136bpm G3 murmur Abdominal palpation- Urogenital- Musculoskeletal- Neck flexion NAD slightly reduced on L Neck flexion. tense through mid lumber spine BCS- 4.5/9 Coat/skin- NAD Palpate lymph nodes- NAD Temp- 38.3 Assessment: sore through mid lumber spine and left flexion of neck Treatment: Gabapentin Meloxicam Next vaccination due: Vaccinate once better Plan: On lead and strict rest Suggested x-rays or CT for further diagnostics
