Appointment Notes: ran over paws and legs with office chair ADDENDUM:  ***Soft systolic murmur noted - when mentioned to owner she indicated she was aware of this/pre-existing but on later review of clinical records could not not find mention of this. Plan to follow up with owner.***Subjective: - Hindlegs caught under wheels of office chair approximately one hour prior to presentation - Was vocalising and limping badly immediately after the incident, has been able to walk since and vocalisation has stopped - Otherwise well - No current medications - Known luxating patella in left hind leg - Known allergy to chickenObjective: - General appearance: appears reasonably comfortable, no distress beyond usual anxiety at clinic. Fully ambulatory with no overt lameness detected. Eats treats enthusiastically. - Mucous membranes: pink, moist, prompt CRT - Heart rate and rhythm: normal. Soft systolic murmur auscultated. - Abdominal palpation: comfortable, no abnormalities detected - Front legs: comfortable on palpation - Spine: comfortable on palpation - Hips: comfortable on palpation - Left hind leg: mild reaction on paw palpation, nothing dramatic; luxating patella present; no evidence of fracture on palpation of distal limbAssessment & Plan:1.
