Appointment Reason: Nail Clip, Appointment Notes: Nail trim recc consult is chewing feeting and legs KS------- Sms reply: YHistory:Presented due to chewing feet and legs in both FL's Seems to be inside of her forearms and around her wrists (less so her toes) When at the park and playing with ball will run and stop and sniff her legs. Then continues to play. Happens daily for > 1 year Not sure whether psychological or true painHas been using pain killer liquid at home - think that's it's been helping - liquid (meloxicam?) - been on it for about 4 days Otherwise BAR, eddu well, no v+/d+, client has no other concerns Brushes her teeth often Parasite Prevention: Nexgard Spectra Diet: HillsMedication: Clinical Exam: *muzzled to examine paws and toes*Demeanour: BAR, anxiousHR: 136bpm, reg rhythm, nmaRR: 24bpm, normal bronchovesicular sounds bilaterally MM pink + moist CRT < 2 secT: DNP to minimise stress Dental Grade: 0/4 BCS: 5/9Eyes: Clear bilaterally Ears: Clear bilaterally Integ: Coat clean - interdigital spaces cleanGIT: Abdomen soft and comfortable on palpation PLN: Submandibular, prescapular and popliteal lns palpable and wnl MSK: Ambulating well, even muscle coverage, tolerated flexion and extension of joints in both forelimbs with no pain or crepitus. Resents palpation of digits both FL's - no swellings or crepitus on palpation of digits. Query very mild carpal valgus Neuro: BAR, normal mentation, full neurologic exam not performed Genitourinary: NAD Lab: Assessment: MSK pain more likely - ddx ligament strain vs anatomic issue in carpus vs OA Treatment: Continue pain relief trial with meloxicam oral 6kg dose PO SID for further 14 days Plan: R/V in 2 weeks Recommended modified rest for the next week - no ball at the park, sniff walks around the block are OK, gentle mouthing ball play on the couch is OK In one week- return to park (still with meloxicam on board) --> monitor if signs are still persisting If improved with rest and meloxicam - more suspicious re MSK pain- consider GA and rads of FL's for further investigation.
