<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>Hx:<br>- Reports ear infection with discharge and irritation<br>- Applied olive oil to affected ear<br>- Symptoms persist despite home treatment attempts<br>- Ear appears inflamed this morning prompting veterinary consultation<br><br>Objective:<br>- Bilateral otitis externa identified <span style="color: #626262; background-color: #ffffff;">- Otoscopic examination performed. </span><span style="color: #626262; background-color: #ffffff;">Cytology positive for yeast on both ears</span><br>- <br>- General appearance: Bright, alert, responsive. Normal body condition score.<br>- Vital signs: Temperature, pulse, and respiration within normal limits. Mucous membranes pink, capillary refill time <2 seconds.<br>-Nose, Throat: Normal. No nasal discharge.<br>- Oral cavity: Normal.<br>- Skin & coat: Healthy, no signs of pruritus or alopecia.<br>- Thoracic auscultation: Heart and lung sounds normal.<br>- Abdomen: Soft, non-painful, no organomegaly palpated.<br>- Musculoskeletal/Neuro: Normal ambulation, no deficits noted.<br><br>Assessment:<br>- Bilateral otitis externa with yeast infection vs - Foreign body remains possible but less likely<br><br>Plan:<br>- Easotic drops prescribed once daily for five days<br>- Review in five days if no improvement<br>- Sedation and further examination may be required if condition persists<br>- Ear care advice provided: regular cleaning with baby wipes once weekly, thorough drying after cleaning to prevent moisture accumulation<br>- Consider elevated water bowl to prevent ear contact with water during drinking<br>- Monitor for any foreign body as contributing factor</p></html>

Hx: Here to be vaccinated. O has no concerns- EDDU normal, no V+, no D+, no C+- UTD with worming/fleas controlPE- Mentation: BAR, lovely dog- CVS: No arrhythmia or murmur auscultated. Pulses strong and strenuous.- Resp: Normal respiratory rate. Normal bronchovesicular sounds bilaterally. No crackles audible. Nil cough or nasal discharge. Normal respiratory effort- CRT/MM: <2secs, pink and moist- Otic: Normal pinnae on basic external aural / otic exam- Ocular: No abnormalities detected. Visual without ocular discharge or grossly evident ocular lesions.- Dental: NAD- GI: No abdominal distension. No abdominal pain. Nil abdominal mass effect appreciated.- Urinary: Bladder palpable and intact and non-painful, nil pertinent urinary tract signs.- Neuro: Normal mentation, gait and awareness. Nil ataxia or conscious proprioceptive deficits. No cranial nerve deficits.- M/Skel: Ambulatory. No spinal or cervical pain. Normal gait and posture. No lameness, joint effusion, crepitus or pain on ROM exercises noted.- Integ: NAD- LNs: No abnormalities detected, nodes palpated as WNLsAssessment- Fit to be vaccinatedPlan- Received C5
