<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><div> <div> <p>BIB son Zac<br>- noticed lump this am, v well o'wise/seems fine - EDUF all ok, v BAR<br>- no vet visit since 2021 visit here, o'due vaccs/etc<br><br>- L neck/submandibular LN area round swelling approx 9cm diam - LN related or salivary gland possibly? lower ventral R neck deep possible lump approx 5cm diam or part of normal, no facial swellings, MM pink/crt ok, chipped RU carnassial, swelling non-painful and discrete/round, not associated with overlying skin<br>- doesn't chew sticks/FBs<br>- heart auscult ok, abdo palp ok, weight increased to prev BCS 5/9<br><br>- lymph node or salivary/sialocoele?/FB abscess, infection/inflammation, etc possibs<br><br>- sedation req'd for FNA so to trial course abs and antiinflams and investigs under sedation if progression/not resolving<br>- postpone vacc/HW for now<br><br>- Amoxyclav 500mg - 1.25 tabs bid 8d<br>- metacam 2.5mg - 2 sid 7d</p> </div> </div></html>

Needs sedation to check around head - head shyBloods -Cre 131 (27-124) suspect pre renal - check USGIV catheter placementSedation Medetomidine 0.01mg/kg (0.5ml) + Butorphanol 0.3mg/kg (1.3ml) IVAlfaxalone top up - refer to GA chart. Soft flocculent 9cm x 6cm mass L mandible. Thick haemorrhagic salivary material aspirated - refer to photo. Placed in urine container, Gel swab + slide. Drained 69ml 18g (1 spot)Checked inside mouth no wounds etc noted. Reversed atipamazole 0.5ml IMElbow hygroma Good recovery. Cytology: Possible bacteria, neutrophil and possible macrophage. IC spoke to Steven 0407 773 771 - 13.05 - updated on findings Explained is mostly cosmetic if not painful etc or can be surgically removed - will need to go specialist and remove both chains generally. Is not sore, doesn't bother him etc. Happy to culture just incase and also extend AB. Hygroma sleeps on the cough or large rub. Will monitor for now. Will conservative manage at this stage. Vax with r/c in 14 days time with r/c. Plan:1) r/c in 14 days time - after AB finishes - if ok then vax2) Pending culture3) Bring urine in for USG ONLY - PAIDTreatment1) Amoxyclav 500mg 1.5 tab BID 14 days

BIB StevenH:Past 2 days has been favouring right fore paw, O thinks may have sore toe. Very active and goes for long daily walks. E:BAR, very excited/vocalheart ausc no murmur normal rhythmmm pinkeyes/ears/nose clean NADreactive on palpation of 4th digit right fore paw, no inflammation/swelling, no crepitus, no wound or evidence of trauma, however is mouthing and turning head when applying pressure to digit. No gross lameness in consult.
