<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"></html> </br><p>HISTORY<br>Presenting for: Shana presented for examination of a nail issue that has been present for 4-3 months.<br>Historical Conditions: Patient has lost weight over the years. Owner reports she used to be much more aggressive during veterinary examinations 3-4 years ago.</p><p>EXAM<br>Wt: 4 kg<br>Behavioural: Much calmer during examination compared to previous visits. Previously was very angry and uncooperative during veterinary handling.<br>Abdominal: Kidneys normal size, bladder empty, everything feels normal on palpation.<br>Integumentary: Mucoid purulent discharge present around affected nail. Nail bed appears red and inflamed. One nail has lost the entire nail structure, leaving exposed nail bed and quick which appears sore.</p><p>ASSESSMENT<br>Nail bed infection with complete nail loss - likely bacterial infection secondary to trauma or other underlying cause.</p><p>PLAN<br>New Meds/Tx:<br>- Long-acting antibiotic injection administered - duration 2 weeks<br>- Nail area cleaned and discharge removed</p><p>Additional Discussion:<br>- Monitor nail daily - should show progressive improvement with less redness and discharge<br>- Squeeze nail bed daily to assess healing progress and check for discharge<br>- Normal nail growth will eventually return from the nail bed<br>- Keep area clean if exposed to dirty, dusty or muddy conditions<br>- No follow-up appointment needed if healing progresses normally<br>- Contact clinic if condition worsens or fails to improve</p>
