Reason: Diarrhoea with vomiting History Has been managed for Atypical Addisons for 2years Sloppy and then firm Whimpering- toilet Passed a stool indoors Small Vx afterwards Small pieces of bone in Vx Last 2-3months, tending towards pica Still drinking larger amounts Some periods of tenesmus without passing anything Current medications Zycortal q28days 0.9ml SQ Prednisolone 2.5mg EOD and 5mg EOD This works out to 0.1-0.2mg/kg Examine BCS: 6/9. 2.8kg weight gain in 12months mm pink, moist, crt <2secs HR 72bpm FP strong and synchronous RR 28bpm Chest clear on auscultation Abdomen comfortable on palpation Temperature: Normothermic Rectal: empty rectum, soft pale brown stool. No blood or mucus Eyes: mild epiphora bilaterally Ears: some hair at opening, no smell or discharge Oral: NAD Dental grade: 2/4 Skin: NSF M/S: NSF Neuro: BAR in consult. Normal cranial n. No CP deficits Eating treats when offered Problem list Atypical addisons Vomiting- single episode Diarrhoea Pica Weight gain DDx Plan GHP bloods with electrolytes Maropitant 26.2mg SQ Pro-kolin Switch to Cortate 0.3-0.5mg/kg/day= 7.5mg - 13mg Reduce dose of prednisolone to 0.04mg/kg/day = 1.0mg
