10/03/2026 Perianal fistulas and chronic colitis, managed with immunosuppression. History of skin allergies. Anxiety, for which he receives gabapentin. Mild-to-moderate periodontal disease (Stage 3/4). Prednisolone 1.25 mg PO EOD Cyclosporine (Neoral) 25 mg PO SID; frozen and given with food. Gabapentin 100 mg PO SID, stopped psyllium 0.25-0.5 of a teaspoon every day, eats once daily at night Plan: stop prednisolone if ok in 4wks, reduce neoral to eod At dental- check ears, pluck if needed

Nurse initial: KJ O called to get new script for Neoral tablets we have not done script since May 2025 apparently has seen Animalius for medicine specialist appointment in the last few months but they cannot send me the report for another 2-3 days... Advised needs consult for new script from us currently. phone cut out, please advise O of the above if calls back.
