<html><meta http-equiv="Content-Type" content="text/html; charset=utf-8"><p>Owner present: June<br><br>History: <br>Small drops of blood in diarrhoea, which started yesterday. <br>Not wanting to eat or drink. Started to eat again last night - eating a little bit this morning - chicken breast<br>V+ 3x bile. No V+ since last night. <br>Playing a bit less, but not too lethargic. <br>No access to poisons or toxins. <br>Normal diet:  sometimes has dry food.  <br>Had some: turkey meat + veggies - maybe this didn't agree with her (?)<br><br>Subjective:<br>- C/O vomiting and diarrhoea with blood since yesterday morning<br>- Vomiting occurred 1-2 times during the day yesterday and once at night; vomit was frothy with no food content<br>- Diarrhoea present throughout the day yesterday with blood drops; stool colour changed from normal to dark green/black, particularly after eating<br>- Anorexia from yesterday morning until last night; started eating small amounts of chicken breast around midnight and early morning<br>- Reduced water intake noted; owner attempted to offer water<br>- Activity level decreased; less alert than normal, not playing with ball as usual, but still walking and greeting people<br>- No known access to toxins or poisons<br>- New food introduced on Thursday: home-cooked turkey mixed with pumpkin skin and vegetables, given with Leica; portion was prepared on Wednesday and fed on Thursday<br>- Normal diet consists of dry food and Leica<br>- Up to date with worming and vaccinations<br><br>Objective:<br>- Temp: WNL<br>- Weight: 3.6 kg (stable from previous)<br>- Hydration: not significantly dehydrated<br>- Mucous membranes: pink<br>- Abdominal palpation: no pain<br>- Dental: malocclusion noted with lower jaw positioned forward; teeth erupting in abnormal position; may require extraction in future but not currently problematic<br>- Rectal exam: NAD<br><br>Exam:<br>BAR<br>FAS 3/5<br>BCS 5/9<br>MM pink and slightly, CRT &lt;2s<br>Dentition - normal adult conformation, grade 0/4<br>Eyes – normal pupil, sclera, iris. <br>Ears – clean, no discharge<br>Integument – good condition<br>Lymph nodes: peripheral LNs palpate WNL<br>Cardiovascular auscultation WNL – HR: NMA, SSFP, normal BV sounds, normal rhythm<br>Abdominal palpation – tense<br>Temp 38.8, rectal exam: brown faeces on glove, watery, no blood<br>Neuro: not ex, normal mentation<br><br>Assessment &amp; Plan:<br>1. Acute Gastroenteritis<br>- PCV 57: concern re AHDS<br>- Anti-nausea injection administered IV<br>- Anti-diarrhoea paste: 2-3 ml PO BID until stools normalise; can give up to 3 ml for severe diarrhoea; can mix with food or give slowly into mouth<br>- Bland diet recommended: chicken and rice; additional gentle recovery food discussed<br>- Hydration support: electrolyte solution to be made according to directions and kept in fridge; give 5 ml every 1-2 hours by mouth; leave bowl available; add extra water to food<br>- Option to take to emergency for 12-24h of IVFT to expedite recovery and prevent further dehydration. O unsure about this. Advised emergency open on the long weekend, go if black/bloody stools recur, blood in vomit, continued V+, continued hyporexia and lethargy. <br>- Appetite expected to improve following anti-nausea injection<br>- Stools expected to normalise over 2-3d<br>- If vomiting recurs tonight or condition worsens, present to emergency for further investigation including possible radiographs or ultrasound to rule out foreign body obstruction<br>- Clinic closed Sunday and Monday; advised to monitor closely<br><br>Additional Notes:<br>- Owner counselled on signs to monitor and when to seek emergency care<br><br></p></html>
