** NURSE: TE REASON: ocular discharge. HISTORY: - R eye d/c yesterday. - D/c looks like "sleep". - Dries to a blacky colour. - Some squinting yesterday. - Better today. - Previous consult noted possible FHV or environmental irritant. - Indoor only cat. - Grooms a little. - Brushed every second day. - Bites at night. - Nocturnally active. - no cat flu signs since been with O but was a rescue from ten lives APPETITE: E+D well. Normal. ACTIVITY: Sleeps a lot during the day. Active at night. Not interested in toys. DIET: - 1 sachet wet food/day. - 40-50g Hills dry food/day. - 1 tsp oatmeal/day. - Gaining weight despite O trying to restrict diet TOILETING: WNL CURRENT MEDICATIONS: NAD. CLINICIAL EXAM: - Mentation: BAR - MM: pink - Oral and Dental Score: Gingivitis on back molars both sides. Rest of mouth normal- G2 - Eyes: Both eyes have chemosis, R > L. Small amount of discharge medial canthus both eyes. Pupils even. Sclera WNL. Fluorescein stain negative for ulcers both eyes. Positive nasolacrimal passage both sides. - Ears: WNL. - BCS: 4/5. - Lumps: NF - Abdominal: WNL - Lymph nodes: Normal. - HR: 200bpm, no murmur. - RR: Normal. - Urogenital: WNL - MSK: WNL. - Skin: Coat healthy, slightly unkempt. ASSESSMENT: - Problem list: - 1. Bilateral chemosis and ocular discharge. Ddx: Feline Herpes Virus (FHV), conjunctivitis, environmental irritants. - 2. Overweight (BCS 4/5). - 3. Mild gingivitis- could also be linked to underlying FHV - 4. Nocturnal activity/poss play aggression DISCUSSION: - Discussed possible causes for eye signs incl. FHV flare-up due to stress (e.g. other cats outside at window). - Discussed weight management. Overweight risks incl. diabetes, cancer, arthritis. - Advised reducing food intake. Plan to reduce dry food to 20g/day, maintain wet food- recommend reweigh in 1mths time to ensure losing weight - Discussed behavioural issues at night (play aggression, attention seeking). - Options discussed incl. timed feeder, Feliway diffuser, blocking bedroom access. TREATMENT: - Prescription provided for Chlorsig 0.5% eye drops- One drop in each eye QID for 5d PLAN: - Start Chlorsig eye drops. - Monitor for improvement/recurrence of ocular signs. - Monitor for sneezing. - Reduce dry food to 20g/day plus wet food sachet. - Re-weigh in 1-2mths. Free weight checks offered. - O to consider behavioural modification strategies for night-time activity. - Further diagnostics for eye issue if no improvement or recurs.
