Reason: Revisit Cytopoint + B12 InjectionAppointment Notes: EW  ------- Sms reply: YHISTORY:- Presented for follow-up regarding gastrointestinal upset and ongoing pruritus.- GI upset started after ingesting roast chicken or pork that someone on the street gave him.- Last bowel motion was formed but soft. Probiotic was stopped by the owner due to concern for constipation.- An anti-nausea injection was administered on 05/03/2026.- Appetite is good- Ongoing pruritus not well controlled with oclacitinib (Apoquel). Owner reports Teddy is still itchy, but less so since the cytopoint injection- Itchiness is worst around the ears, also chews front paws.- Diet consists of Prime 100 sausage at night and has recently included more Prime 100 dry food for training purposes.- Flea and tick prevention is overdue. Last administered in January 2026.EXAMINATION:Demeanour: BAREyes: Bright and clear with no discharge Ears: Bilaterally pinnae are clean, however, some erythema noticeable on concave surface.Oral: Gums pink, hydrated. Mild tartar buildupCardiovascular: No murmurs or arrhythmias.Respiratory: Normal lung sounds in all fields.Abdomen: Soft on palpation, although did cry with repeated palpation. Integument: Appears normal.MSK: Ambulating normally.Urogenital: Externally NAD. Rectal exam - soft faeces.LABORATORY:None todayASSESSMENT:1. Suspected irritable bowel disease/food sensitivity.2. Atopic dermatitis, partially responsive to current therapy.TREATMENT:1. Vitamin B12 0.5mL SC2. NGS administered POPLAN:- Recommence probiotics.- Continue oclacitinib (Apoquel) as previously.- Recheck in a week.
