Reason: Recheck +/- BandageAppointment Notes: Overdue remindersHistory: Unsettled following surgeryExamination: P2 swollen and bruised Large amount of blood drained from medial junction between skin and pad when manipulating Wound itself is holding together, doesnt look amazing Sore to palpate P2 Assessment: Ddx: abscess vs haematoma vs degloving injury?Plan: Elected to rebandage, think will just chew at wound if left open Add NSAIDs for pain relief/reduce swelling. Add losec to reduce risk of GI ulcer Recheck + bandage change Monday--- 07/03/2026 5:08:27 PM MR:Bled through banadage, nurses rebandagedNot actively bleeding--- 09/03/2026 11:16:57 AM CDR:Lj attached email from owner. Hi Dr Michael, Thought id give you an update on Nancy. Her movement has been a lot better and she is weight baring on her foot a little more. she's settling easier too. Unfortunately she vomited Saturday night, and sunday too. Sunday breakfast @ 9am vomited around 1pm and dinner @ 7pm vomited 11pm (and the vomit is just once and suddenly too almost like projectile there was a tiny bit of blood but small). she wants to eat straight after. i stopped giving her the anti-inflammatory today and didnt give her any meds last night or this morning at all besides the losec. thanks Nicole. call me if you wantg me to restart meds 0414305696--- 09/03/2026 6:24:15 PM MR:Discussed with O had vomit couple of times over weekend (possibly regurg, doesnt sounds active). Likely from NSAIDs as had no issues previously. Was booked to change bandage today, but had to reschedule. Booked for tomorrow morning. Appears more comfortable. Stop NSAIDs and can hold on panadol if comfortable. Needs to keep having antibiotics Vital Signs
