09/03/2026 Been off her food a couple of days and coughing badly last night. Everyone had a bad night.Taking heart meds ok. Inbetween coughing she acts as normal and runs around the house, playfully like usual. O worried last time she had a bad night and was off food that she had a uti so wanting to get onto it quickly this time. No symptoms of dysuria though. Coughing ++ during consult with quite a wheeze and a swallow at the end. PInk mms, crt 1s-still tries to bite!, lns ok, hr 144, g5/5, crackles evident ventrally bilaterally, still has strong femoral pulses, no ascites, vulv alooks ok, t-38.6. Will take liver treats between coughing bouts. No evidence of uti but after discussion agreed to try convenia again as usually has v good/quick response to it. Discuss my concern that she is now in heart failure although could just be exacerbation of lower airway disease. Declined radiographs. Discuss tx options of diuretics (to reduce pulmonary oedema) v steroids to reduce airway inflammation and side effects of both -PUPD. Discuss possibility that this could be the beginning of the end if symptoms don't improve in the next 24-48hrs Convenia inj and oral flusemide Sandra will be away so contact John re response to tx. cc tomorrow
