HISTORY: Presenting Complaint: Increase haematuria and continued stranguria - Seen 2 days ago with stranguria & haemauria --> started on Meloxicam & Temgesic - Still happening and urine seems more haematuric - Still otherwise well Meds: Meloxicam PO SID & Temgesic PO BID PHYSICAL EXAMINATION: Mentation: BAR ABD. PALP: Comfortable, empty bladder, not blocked **CARDIAC: G1murmur noted today (stress?) RESP: NAD ASSESSMENT: DDX: FIC vs UTI vs STONES COMMUNICATION: - Discussed assessment with owner, recommended further investigation. O consent. TREATMENT: Gabapentin 60mg PO given in hospital this AMDIAGNOSTICS: 1. Bladder ultrasound Bladder wall ~0.18cmHyperechoic material in center of bladder, mobile, likely blood clotNil obvious shadowing to indicate stones 2. USG/Dipstick/Sedivue Suspect Rods 3. Vetnostics C&S Pending resultsASSESSMENT: FLUTD/FIC + UTI COMMUNICATION: JR STO OTP Advised of findings and assessment & plan PLAN: - START Amoxyclav 250mg x 1/2 tab PO BID for 5 days - START Cystophan as per box directions- Await Urine C&S results - CONTINUE on all other medications - RC: INI / 1-2 weeks post abx. JR

HISTORY Vaccine this am. v+ food 2 x this afternoon. Circling/uncomfortable since vaccine. Red nose. EXAMHR 220 but stressed. No arrhythmia. Lungs clear and not dyspnoeic. mm pink with normal crt.LNs normal. Abdomen palpates normally. temp 39.7No other abnormalities. ASSESSMENT Probable mild vaccine reactionLABORATORYTREATMENT Niramine 0.5ml s/c.Prevomax 0.5ml s/c. PLANOWNER COMMUNICATIONMonitor.
