08/03/2026 REASON - Tummy issues. Frequent attempts to poo. Loose poo SN 8/3 HISTORY: 2 nights up in the night throughout the day yesterday: not watery diarrhoea but loose/ soft poo real straining and walking around a lot agitation, pacing going out in the rain no blood or mucus look appetite haven't fed this am ate dinner normally last night not normally a scavenger, do have rabbit around the backyard: poo no V demeanor rather normal gave chicken wing wednesday and yesterday morning: has had that regularly so far: BIW prior to this last last yr GH visit: scooting, had anal gl expressed, 2nd time glands ere fine but inflam around bottom: cortisone cream which also used last night do sometimes attempt to scoot but single use of cream seems to settle. Hence additon of sweet potato to diet Diet: Blackhawk kibble + roast sweet potato BID Prophylactics: Bravecto chew + milbemax tab q3m EXAM: BAR, BCS: 4.5/9, MM: pink CRT: 1-2sec RR: 24/min HR=PR: 96/min auscultation: NAD T: 38.5 Dentition: Grade 0/4, Eyes/Ears: NAD, LNN's: all peripheral ln wnl. Abdo: comfortable, slightly fluid GI feel, no guarding or mass effect. Rectal exam - colon empty, no blood, no mucous, no stools, not straining against examination, anal glands unremarkable MSK: NAD Integument: NAD, Genitourinary: NAD ASSESSMENT: Diarrhoea appears to be large bowel: ddx: dietary indiscretion/ interalance vs all TREATMENT: Pro-Kolin 6mL BID per os until finished Cerenia 60mg 2 SID per os for up to 2 doses PLAN: Simplified diet, small frequent meals 3-4 per day, no treats or extra tidbits or high fat items.
