History: Neptra worked well last time, but o did not clean ears after last visit. Now head tilt to R side
EDDU normally, no other concerns
Vital Signs:
All vitals wnl
Dehydration assessment: Euhydrated MM moist no skin tent
PHYSICAL EXAMINATION:
Appearance: BAR; BCS 3/5
Eyes: NAD
Ears: moderate greenish-brown, ceruminous deposits bilaterally
Cytology: R ear- yeasts +++
L ear- yeasts +; rods +++
Nose: NAD
Oral Cavity: not assessed
Cardiovascular: Auscultation no murmur detected ,pulse quality normal ,rhythm regular
Respiratory: RE normal , auscultation normal
Rectal exam: Both AG filled- expressed normal contents
Assessment/Problem List: bilateral OE
Treatment: GKCT 1.5ml in both ears
Plan: Repeat GKCT for both ears next week (o paid for it today, labelled and kept in fridge- no charge next week)
Recheck cytology week after that
