Reason: Lethargic + High Resting RRHistory: Breathing has gotten progressively worse through the day today. Much worse this afternoon. Not eating at all. Seems very flat and lethargic. Examine: On arrival markedly dyspneoic and tachypenoic, RR approx 120. Short shallow breathing. CV - grade III/VI murmur. Chest - not overly crackly etc. Pulses strong. MM muddy. CRT 1-2sec. Abdomen normal. Lymph nodes mostly normalGrade 3 dnetal diseaseBT 39.5Assessment: CHF vs other resp compromise. Pyrexia does not fit with CHF? Neoplasia vs infectious? Not able to provide treatment or work up here as presented right on 7pm. Referral to SASH tonight. No futher work up done today.    Vital Signs
