Reason: Infected Torn Dew ClawHistory: When returned home from a walk 5 days ago noticed Wes had torn off his dew claw. Seemed alright initially but now looks a bit swollen and wondering if infected. No lameness and not licking at it. Has been lethargic also. Normal EDUF. No vomiting/diarrhoea. No coughing/sneezing.Has been scratching at his ear. Diet: kibble + chicken mince/meat Preventatives: nexgard spectra UTD Medication: cortisone acetate 2.5mg PO BID, flornief 0.2mg PO BIDObjective: Quiet, alert and responsive. Weight loss of 120 grams. Eyes/nose/mouth: NAD. Ears: NAD on external & otoscopic examination. Integument: left dew claw has been torn off, quick exposed. Nail bed swollen & erythemic. Thoracic auscultation NAD. Abdominal palpation NAD. Peripheral lymph nodes NAD. Trimmed remaining nails. Assessment:Infected nail bed post traumatic tear of left dew clawPlan: Clean with dilute chlorhexidine BID (owners have at home already). Commence amoxicillin clavulanate 300mg PO BID. Will not place E-collar today as no self trauma but can provide for owners if required moving forward. Will not bandage at this stage as not weight bearing, and not actively discharging currently. Recommended increase dose of cortate to 5mg tonight given stress of rhe appointment today, then can reduce back to maintenance dose of 2.5mg PO BID ongoing.
