Reason: Recheck, See NotesHistory: Hasn't vomited all weekend (since friday) however has now been leaking faeces.Thinks faeces are softer than normal, but also defecating at random times. Also scooting bum across floor and furniture.Eating well still, still seems bright within herself. Changed diet completely from lyka to i/d food on friday.Finished zofran/metoclopramide/omeprazole tabs. Haven't given any all weekend.Examine:BAR.CV: normal rhythm, rate & regularity.RESP: normal lung sounds all fields.EYES/EARS: clear, no discharge.INTEGUMENT: perianal region mild erythema from scooting. Surgical site looks good, no swelling or erythema. Healing nicely.ABDO: difficult to assess w/sutures but seems relatively comfortable.Temp 38.7Assessment: colitis/GI sensitivityPotentially worsened by diet change on friday vs ongoing gastroenteritis post gastric lavage/gastrotomy. Treatment:Milbemax x 1 tab PO.**later vomited this tablet up in the waiting room, gave O another tablet to give to Luna when gets home with something to eat. Plan: Continue i/d diet - gave more can food as well as dry. Metoclopramide (10mg) 1mg/kg (1tab) PO BID.If needed - ondansetron (8mg) ~0.5-1mg/kg PO q8-12hrContinue prokolin 3ml PO BID until d+ resolved.If still has d+/ "faecal incontinence" tomorrow, RV.
