Bilateral otitis externa - chronic issue. 7 days of topigen (o has released she might be using slightly more than 0.5ml per ear, as she has run out after 7 days). 

cytology shows ear wax only, no cocci or malassezia present. otoscope shows no stenosis, a small amount of ear wax ball in both ears, and some scarring of the L ear near TM. both TMs in tact. 

no further drops required. O to use regular ear cleaners when at the beach or wet/humid times, and otoflush with dex once per week. 

recheck if ears get worse again, next step would recommend sedation ear flush to remove the wax balls in his ears.
