Appointment Notes: Poss uti History:Presenting for dribbling urine. Has had multiple UTIs in the past. Last year had 3-4 over a 3 month period, treated with antibiotics. Then it completely stopped and had none this year. Mostly today (could have been yesterday, O unsure wasnt paying attention yesterday), was dribbling urine, might have strained as well trying to urinate. Has blood in the urine. EDDU: Normal besides urination. No V+/D+/C+/S+. Examine: 1. BAR2. Vitals: NAD, CRT < 2s, mm pink and moist.3. Body condition: 5/94. Oral exam: NAD. Moderate to marked amount of tartar/plaque/calculus on all teeth. Moderate gingivitis.5. Eyes: NAD. Both visual, cornea transparent, no blepharospasm/epiphora/discharge/hyperaemia.6. Skin / ears: NAD. Ear free of discharge/odour. Good coat condition.7. Auscultation: NAD. No heart murmur/arrhythmia. Clear lung fields. Normal bronchovesicular sounds. No wheezing/crackles.8. Abd palpation: NAD. Bladder almost empty. Palpation of abdomen seems sensitive. 9. Palpable lns: NAD. No enlarged LN, symmetrical.10.
