<p>PRESENTING COMPLAINT: <br>- Pruritic, scratching constantly<br>- Taking fur off ears, ears matted<br>- Scratching hind legs and ears particularly</p>
<p>HISTORY:<br>- Started when stayed with neighbor for a week<br>- Had just been groomed before staying with neighbor<br>- Constant itching since returning home<br>- Scratching hind legs and ears, between legs<br>- Seeking cool spots, going under sofa<br>- Not urinating normally at night walks<br>- Long walk yesterday was normal<br>- Never had issues like this before<br>- Fine when given treats but not interested in usual dinner</p>
<p>Weight: 9.8 kg<br>Temperature: 38.4</p>
<p>EXAMINATION: <br>Eyes: mild discharge bilaterally<br>Ears: quite inflamed, red, small amount of dirt, tm intact<br>MM: pink and moist, crt &lt;2<br>Heart and chest sound fine<br>Coat &amp; Skin: mod inflammation in medial thighs - hot to touch, red, moist<br>Otherwise NAD<br>Scratching evident in consult</p>
<p>PATHOLOGY: ear swab and skin smear taken - mild bacterial infection, no Malassezia</p>
<p>PROBLEM LIST:<br>- Severe pruritus<br>- Inflamed ears<br>- Skin inflammation between thighs<br>- Secondary bacterial infection</p>
<p>DDx:<br>- Allergic dermatitis<br>- Contact dermatitis from grooming products<br>- Environmental allergens<br>- Secondary bacterial infection</p>
<p>ASSESSMENT: Severe pruritic episode with secondary bacterial infection. Likely allergic reaction possibly triggered by grooming products or environmental exposure. Needs immediate anti-itch treatment and topical therapy for infection.</p>
<p>TREATMENT in consult room/Meds dispensed:<br>- Cytopoint injection given - 10mg<br>- Topical ear/skin medication dispensed - PMP for 10 days<br>- E-collar fitted</p>
<p>VET TECH actions:<br>Generate client summary in VetNotes <br>Email handout: none needed</p>
<p>PLAN:<br>- Apply topical medication to ears BID and inflamed skin areas for 10 days<br>- Keep e-collar on for 10 days<br>- Monitor improvement over next 2 days<br>- Avoid grooming and bathing for now<br>- If no improvement in 2 days, return for further investigation including possible skin scrape<br>- Vaccination due this month - postponed due to current condition</p>

Consulting with Vet Nurse: noPRESENTING COMPLAINT: - Vomiting for several days after eating rawhide- Brought in by Os neighbourHISTORY:- Started Friday/Saturday when Lt rawhide, found vomit on floor that looked like chewed up rawhide- Continued vomiting Sunday morning, stopped regular food, gave boiled rice and chicken soup- Monday kept same diet, seemed well in himself, walking fine- Vomiting again this morning (bile puddles)- Softer stools but no diarrhoea. Stool slightly darker.- More grass eating than usual- Behaviour change - pulling on lead, whimpering in car (unusual for him)EDDU: Keeping down water, softer stools, increased grass eating, vomiting bileEXAMINATION: MM: pink,moist, CRT <2Dental: mod tartar generalisedHeart and chest fineAbdomen tense on palpationL ag full on rectal palpation, dark faeces in rectum, softCoat & Skin: neg skin tentDemeanour: BAR in consultPROBLEM LIST:- Vomiting post rawhide ingestion- Soft stools- Dark faeces- Behavioural changesDDx:- Foreign body obstruction (rawhide)- Gastric irritation/ulceration- Upper GI tract bleedingASSESSMENT: Ongoing vomiting following rawhide ingestion concerning for possible foreign body obstruction. Dark faeces may indicate upper GI bleeding/gastric irritation. X-rays recommended to rule out obstruction. Rang Archie's owner to discuss estimate, recommended at least xrays and fluids +/- bloods. All declined by O as she thinks he just has a sensitive stomach. Explained dark stool concerning as well as duration of vomiting episode. Explained that if left, could worsen prognosis.TREATMENT in consult room/Meds dispensed:Gave 1ml prevomax inj scDispensed: 2 x cerenia 24mg tabs SID, 10 days losec BID, 1 prokolin dispenserVET TECH actions:Generate client summary in VetNotes Discuss estimate: X-rays recommended to check for foreign body obstructionEmail handout: none neededPLAN: - Monitor at home for ongoign vomiting, if vomiting through cerenia come back.
