Reason: Health Check, AG's & Nails.Appointment Notes: PAHistory:Presented for an eye and anal gland check.Lump on the left eyelid, present for some time.Examination:Eyes: 3 mm hard, non-tender lump on the medial canthus of the lower left eyelid. The lump has some external secretions and crusting. It is located directly over the lower lacrimal punctum.Dental: Grade 1 dental disease.Chest auscultation: Grade 1 heart murmur detected. Heart rate 130 bpm.Rectal examination: right anal gland was slightly distended, with a thickened and leathery wall, approximately 15 mm x 10 mm. The gland was fully expressed, yielding a mix of watery and brown, slightly grainy secretions. left anal gland unremarkable.Assessment:1. Left Eyelid MassLikely meibomian gland adenoma.2. Cardiac MurmurGrade 1 heart murmur. Possible early degenerative valve disease or stress-related.3. Anal Gland ImpactionLeft-sided anal sacculitis.Treatment:Manual expression of the left anal gland performed.Plan:Monitor the eyelid lump. Advised against removal at this stage due to the risk of damage to the tear duct.Continue three-monthly anal gland checks.Monitor the heart murmur at subsequent visits. gr1 murmur was not mentioned or discussed with client today Vital Signs
